New Patients
If your pet will be coming for his or her first visit, please download and bring the completed new patient registration form to your appointment. If your pet has a previous veterinarian, please have his or her records faxed to us at (914) 834-0310 prior to your appointment. If you will be bringing your records with you, we kindly ask that you arrive 10 minutes prior to your appointment time to allow us time to copy and review your pet’s medical history.
